The application form you need to fill
in will depend on who you’re applying for.
- If the pension is for you, another adult or a
child over 16 and in full-time employment, complete the
adult application
form
- If the pension is for a child
currently under 16 or under 18 and not in full time employment,
complete the
child application
form
For both forms you’ll need your bank details
and the applicant’s National Insurance number to hand.
